SOCSTC's Wrap Around Services Program serves youth and families at risk for out-of-home placement or incarceration, then assists in transitioning them back to the community from residential placement.

Approach
The Wrap Around
Services Program uses the wrap around process to help children, youth, and
families with complex needs develop an individualized plan of care and to
provide support in accomplishing identified goals. Treatment is based on
the identification of needs, strengths, values, and preferences of the youth,
family, and community, and then acting to provide for meeting those needs.
